What is Case Management?
- Case Management works with the individual on a one-to-one basis to assist him or her to live as independently and successfully as possible.
- It is a collaborative process between the individual and the Case Manager which includes assessment, goal planning, coordination and evaluation of service.
- Case Management works in close partnership with other community agencies and professionals to ensure a coordinated and comprehensive approach.
